Mosaic in Lubbock partners with people with intellectual disabilities to provide a meaningful life in a caring community, giving a voice to their needs.

 

Mosaic provides a life of possibilities through supports which are tailored to each person's needs and desires.

Mosaic in Lubbock offers the following supports:

Home and Community Based Services (HCS)

Residential Services (HCS) - We assist people living in group residences with socialization or daily living activities such as money management, cooking, shopping, etc. This is accomplished by identifying the level of support that is most appropriate for the person and their needs, and ensuring that trained professionals are on-site to provide those supports.


Foster Care (Host Homes)
- Host homes match a person receiving services with someone who provides the assistance needed for daily living in a family-like or roommate-like setting. The program is designed to allow a person to enjoy the benefits of family living while continuing habilitation and personal growth.

Respite Services (HCS, TxHmL) - Providing assistance to full-time caregivers so that they may take a break, respite services are provided through trained and supervised Mosaic staff within the individual's home or through temporary placement of the individual within a Mosaic community-based home or a foster care home.

Supported Home Living Services (HCS, TxHmL) - We link services and supports with existing natural supports and generic community services so that adults eligible for Medicaid who live with family, or are responsible for their own living arrangement, have the necessary supports to be included in typical community life. Supports may include items such as grocery shopping, assistance with medications, medical care coordination and group activities.

Intermediate Care Facilities/Developmental Disabilities (ICF/DD) Homes - Mosaic operates six-bedroom homes where staff and nursing care are available and provide full-time care and supports.

Nursing (ICF, HCS, TxHmL) - We offer professional nursing services and oversight of medical care, as needed to each person Mosaic supports.

Expressions Day Habilitation (ICF, HCS, TxHmL) - We provide opportunities for activities that encourage autonomy, independence and opportunities to establish relationships and make positive contributions to the community. Activities include games, crafts, classes, exercise and engaging events that are similar to a retirement program or extracurricular class.

Private Pay Services - Supported home living, respite care and the Expressions Day Habilitation program are all offered as private pay services for people with intellectual disabilities within our community.
