Mosaic in Connecticut partners with people with intellectual disabilities to provide a meaningful life in a caring community, giving a voice to their needs.

Mosaic in Connecticut is the recognized leader in making a positive difference in people’s lives. We are passionate about services and partnerships that create meaningful lives in caring communities. Throughout 20 communities in Connecticut, people with disabilities live in a variety of residential  arrangements and some individuals participate in Individualize Day Support. We are successful with the help of our well-trained professional staff who partner with each person we serve, their family members or guardians, and other important people in their lives.

Some of the supports that Mosaic provides include but not limited to the following:

Community Living Arrangements
Mosaic’s attractive residences, located in many Connecticut communities, include a variety of residential supports to individuals according to each person’s unique needs. Services provided in these group home settings range from 24/7 assistance with all aspects of daily living to a variety of opportunities for increased independence.

Individuals who receive supports in our CLAs may become active members of their communities, obtain gainful employment, volunteer, attend church and much more.

Individualized Home Supports
 This setting provides people the supports they need to live their most independent lives. Individuals with intellectual disabilities who reside in their own homes, apartments, or with their families may receive Mosaic’s personalized services, including assistance with medical appointments, banking, shopping, preparing meals, and accessing resources in the community.

In-Home Day Supports
Mosaic coordinates a range of services for people with intellectual disabilities who have chosen to retire or whose medical conditions make it difficult to leave home on a daily basis. Activities are customized to the individual’s interests and preferences. In-home activities include arts and crafts, games and puzzles, cooking and projects. Community activities fulfill the unique social and leisure time preferences of each individual through recreation and organized outings in their own communities and beyond.

Community Companion Homes  
Living in a shared residence, an independent contractor provides services designed around the person served, fully integrating them into the life of the household and the greater community. Mosaic provides quality assurance, ensures that mandatory health care requirements are followed, and provides training and annual planning so that individuals living in CCHs reach their full potential and realize their goals and dreams.

Mosaic Dream Catchers
Since 2008, this self-advocacy group for the individuals we serve has empowered people with disabilities in Connecticut by giving them a voice in their communities and with legislators. Mosaic Dream Catchers educate others about their rights and lobby for the passage of laws.
